Payment for Services

The center accepts cash, personal check, debit and credit cards. The policy of this center is to collect payment in full at the time service is rendered. The center does not accept insurance for payment; however, if requested, you will be provided with a properly coded, itemized receipt that includes the appropriate diagnostic and billing codes. Many insurance plans will cover part of the cost of office visits according to their out-of-network provider schedule, and may cover some or all of the costs of laboratory testing.

For your convenience, you may pre-pay for your office visit or any service or product offered by Dr. Perry by using the "Pay Now" button below. You do not need a PayPal account to use this service. Medical Services

Please be advised that a $25 fee will be charged for appointments that are not canceled prior to twenty-four hours before the scheduled date of service. There will be a $15 charge for returned checks.
